We inform you that in accordance with the order of the head of the Siverskodonetsk City Military Administration dated 11/29/2024 No. 1010 VA “On the termination of the municipal non-profit enterprise “City Dental Polyclinic” of the Siverskodonetsk City Council by joining the municipal non-profit enterprise “Consultative and Diagnostic Center” of the Siverskodonetsk City Council”, the procedure for terminating the enterprise has been initiated.

Within the framework of the specified procedure, a reorganization commission (hereinafter referred to as the Commission) was created, which carries out measures related to the termination of the activities of the Municipal Dental Polyclinic and its accession to the CDC.

At the same time, we inform you that in accordance with the Law of Ukraine dated 01.05.2025 No. 4412-ИХ and Article 13 of the Law of Ukraine dated 15.03.2022 No. 2136-ИХ, the period of suspension of employment contracts has been extended for the period of martial law (in accordance with the Law of Ukraine dated 14.01.2026 No. 4757-ИХ, the period of martial law has been extended until May 04, 2026).

After the termination or cancellation of martial law, the employment contracts will be fully reinstated, and employees are required to arrive at the place of work designated by the employer to perform their duties. Failure to arrive without good reason may be considered a violation of labor discipline in accordance with the legislation of Ukraine.

By agreement of the parties, the period of suspension of the employment contract may be extended beyond 90 calendar days (but not longer than until the termination or cancellation of martial law) based on a written application by the employee.

In connection with the above, we ask employees of the Municipal Dental Clinic, including those with whom there is no contact, to contact the Commission or the administration of the Municipal Dental Clinic by May 4, 2026 to determine further employment relations and complete personnel documents.
